救急医学における頭痛―脳血管障害，中枢神経感染症による頭痛を見逃さないためには？

ポイント

頭痛の診断にあたっては“Assume the worst”の原則に基づき，脳血管障害(特にくも膜下出血)，中枢神経系感染症の鑑別に注意する．

CTはくも膜下出血をはじめとする脳血管障害，脳腫瘍，副鼻腔炎の診断に有用である．

臨床的にくも膜下出血が疑われるが，CT所見が陰性の場合,さらに腰椎穿刺を行い血性髄液の有無を確認する．

MRIは，ほとんどの脳腫瘍，等吸収の硬膜下血種などにおいてCTより感度がよいとされるが，急性期のくも膜下出血については，CTを上回るものではない．
