急性呼吸窮迫症候群(急性呼吸促迫症候群)

　急性肺損傷(ALI)および急性呼吸窮迫(促迫)症候群(ARDS)は，何らかの基礎疾患の経過中に急性に発症し，治療抵抗性の呼吸不全および胸部画像上両側肺に浸潤影を呈する疾患である．左心不全は否定される．ALIとARDSは同じ疾患概念であり，ALIの重症型がARDSである．PaO₂/F_lO₂(P/F比)が300以下をALI，200以下をARDSと定義する．基礎疾患として頻度の多いのは肺炎や敗血症である．本症の基本病態は肺血管透過性亢進型肺水腫であり，全身性炎症反応症候群(SIRS)に基づく多臓器不全の一症状であるとされている．本症の発生機序は十分に解明されていない．したがって，根本的な治療法がなく死亡率が高い．最近，低容量人工換気療法，重症敗血症に対してヒト活性型プロテインＣ，好中球エラスターゼ阻害薬が多数例による多施設臨床試験で有用性が確認された．
