胆管結石症の内視鏡治療

ポイント

胆管結石の大部分は経乳頭的に截石可能である．

ESTでは出血，穿孔が，EPBDでは急性膵炎が懸念される重篤な偶発症である．

経皮経肝的截石術は巨大結石例や経乳頭的截石困難例が適応となる．

腹腔鏡下手術は一期的に胆囊・胆管結石治療できる長所を有する．

多種多様な胆管結石すべてを腹腔鏡下手術で截石することは，現段階では議論の余地がある．
