慢性心不全

ポイント

心不全には神経体液性因子が関与している．

NYHA重症度分類による心不全治療薬の選択を行う．

収縮機能障害だけでなく拡張機能障害も評価する．

無症状期からの治療が推奨されている．

ACEI(またはARB)とβ遮断薬が慢性心不全治療の中心である．

β遮断薬導入は少量から開始し，心不全徴候がないこと確認し漸増していく．

利尿薬やACEIまたはARBを使用する際は電解質異常や腎機能悪化に注意する．
