急性心筋梗塞に対する血栓溶解療法―tissue-type plasminogen activator(t-PA)を中心に

ポイント

血栓溶解薬は遺伝子組み換え型の第2世代のt-PAが主流である．

ACC/AHAガイドラインでは，①ST上昇を有し発症12時間以内の75歳未満の症例，②急性心筋梗塞を疑わせる脚ブロック症例，が血栓溶解療法の最もよい適応とされている．

PCIが普及した本邦における血栓溶解療法の位置づけは，①PCIが施行可能な施設に移送するまでの補助的治療法，②予後改善効果を期待したPCI施行前の薬物療法，と考えられる．
