まず考えるべきこと：右室拡大からどのような病態・疾患を念頭に置くか

■右室拡大の原因疾患

　右室拡大は，①右室容量負荷，②右室圧負荷，③右室心筋障害・右室収縮能障害により生じる(図1，表1)．


右室容量負荷

　右室容量負荷をきたす疾患には，心房中隔欠損症・心室中隔欠損症・心内膜床欠損症などの左右短絡性疾患と，Ebstein奇形などの三尖弁逆流および肺動脈弁逆流を認める弁膜疾患がある．
