ポイント

●悪性の頭痛では，「くも膜下出血(SAH)」と「髄膜炎」から考える．

●「突然の痛みだった」「今までに，こんな頭痛は初めてです」(sudden severe headache)なら，くも膜下出血を疑い，脳外科にコンサルトする．

●くも膜下出血は，CTだけでは除外できない．

●髄膜炎疑い例の髄膜刺激症状の診察は，感度と特異度を理解する．

●片頭痛の診断は，国際頭痛分類第2版 (ICHD-II)を理解する．
