ステロイドを必要とする感染症―「困ったときのステロイド」では，困ります

ポイント

●ほとんどの感染症にステロイドは必要ない．特に診断が確定していないときに安易にステロイドを用いてはならない．

●低酸素血症を伴うカリニ肺炎(ニューモシスチス肺炎)には抗菌薬とともにステロイドを併用する．

●細菌性髄膜炎には抗菌薬処方「前」にデキサメタゾンを用いる．

●結核でステロイドを使うのは，心外膜炎と髄膜炎のみである．

●ARDSでは，ステロイドを使用しないのが原則である．
