好中球減少患者/化学療法中の患者での肺炎―Q. 他の患者群での肺炎と異なる対応が必要か？

ポイント

●発熱性好中球減少症はmedical emergencyであり，救命のためには早急な治療開始が必要である．

●好中球減少患者であっても，わずかな症状所見を見逃さず，フォーカスを絞る努力が必要である．

●発熱性好中球減少症で早期に問題となる微生物は，Pseudomonas aeruginosaを含むグラム陰性桿菌である．重篤な好中球減少が遷延する場合，CandidaやAspergillusなどの真菌が問題となる．

●侵襲性肺アスペルギルス症の診断には，早い段階での胸部CTが有用である．
