急性期の再灌流療法―血栓溶解療法とPCIはいつ，どこまでやるのか

ポイント

●急性心筋梗塞(AMI)に対する血栓溶解療法や経皮的冠動脈形成術(PCI)などの再灌流療法が急性期に行われるようになり，死亡率の低下および長期予後改善効果が示された．

●ST上昇型急性心筋梗塞に対してはステントを用いたPCIが第一選択とされ，適応は発症から2日以内であれば有効である．

●非ST上昇型心筋梗塞あるいは不安定狭心症に対しては，早期に冠動脈造影を施行し，責任血管にPCIを抗血小板薬使用下に施行するが，場合によっては外科的手術も必要である．

●drug eluting stent(DES)の急性冠症候群に対する使用は，再狭窄減少という効果はあるが，晩期ステント血栓症のリスクがあることから慎重でなければならない(わが国では保険適用は認められていない)．
