ステロイド服用中のリウマチ疾患患者が結核を発症した場合

ポイント

●結核治療においては，キードラッグであるリファンピシンとの薬物相互作用が鍵である．その数は多種多様なので，必ずそのつど確認する必要がある．

●プレドニゾロン換算で15mg/日が結核発症のリスクになる1つの目安である．

●TNF阻害薬を併用している際は，ツベルクリン反応陰性でも結核を発症しうるため，慎重な経過観察が必要である．また，TNF阻害薬投与中の結核は約半数が肺外結核である．

●ステロイド以外の薬剤(特に免疫抑制薬)，糖尿病・腎障害などの併発症なども感染症のリスクとなるため，十分注意する．
