救急室にきた腹痛高齢患者への造影CT検査

ポイント

●高齢の腹痛患者は，重症疾患のことが多く，原則的には入院する．

●バイタルサインは必須，高齢者で血圧正常(偽性正常血圧)を示すときは重症疾患が隠れている．

●造影剤投与の前に確認すべきことは腎機能，アレルギー反応の既往，内服薬である．

●糖尿病＋腎機能障害(血清クレアチニン1.5mg/dl以上)は，造影剤腎症の危険因子である．

●必要があれば，腎機能障害でも造影剤を使用するが，その後の経過観察が重要である．
