抗アルドステロン薬

ポイント

●抗アルドステロン薬は，降圧・利尿作用を超える臓器保護作用を含めた多彩な薬効（pleiotropic effects）がある．

●アルドステロンブレークスルー現象を考慮すると，ACE阻害薬やARBとの併用は特に有用である．

●腎保護作用がある一方で，腎機能低下例では高K血症をきたしやすく，注意が必要である．

●エプレレノンは受容体選択性が高く，女性化乳房などの副作用が少ない．

●アルドステロンの作用は，食塩環境と密接に関連しており，塩分摂取制限が重要である．
