β遮断薬：心不全

ポイント

●慢性心不全の治療目標は，予後とQOLの改善である．

●日本で使用できるβ遮断薬で，慢性心不全のガイドラインで推奨されているものは，カルベジロールとビソプロロールのみである．

●症状がある心不全症例のみでなく，症状がない心収縮能低下症例や，心収縮能の低下がなくとも心筋梗塞の既往がある全症例に対しても，投与が推奨されている．

●β遮断薬導入によって，一時的にではあるにせよ血行動態の増悪する可能性があり，注意が必要である．

●合併症のある症例では，注意して投与する必要がある．
