ポイント

●潜在結核の診断は長年ツベルクリン反応に頼ってきたが，日本ではBCG接種による偽陽性が問題であった．QuantiFERON^®-TB（QFT）検査はこの問題点を解消する可能性がある．

●QFT検査は結核感染の既往があれば，活動性がなくとも陽性になりうる．QFT検査が結核の活動性の有無を示すというのは誤解である．

●QFT検査は活動性結核に対しても80％弱の感度であり，陰性でも結核感染を除外できない．潜在結核に対する感度はreference standardが存在しないため，厳密には不明である．

●潜在結核治療の際には，必ず活動性結核を除外しなければならない．
