ポイント

★ワルファリンはさまざまな因子に影響され，PT-INRによるモニタリングが必須となる．

★本邦では若年者（≦70歳），高齢者（＞70歳）で至適INR値が異なる点に注意が必要である．

★ワルファリン導入はDaily dose法のほか，患者の状況によって外来導入も可能である．

★出血リスクには，高齢者やPT-INR高値のほか，脳梗塞既往，高血圧，転倒などがある．

★ワルファリンによる抗凝固療法はオーダーメイド治療として扱わなければならない．
