ポイント

★脳梗塞の急性期診療は血栓溶解療法であるt-PAの使用を念頭に置いて，神経所見と画像を有効に利用して短時間で診断からdecision makingが行われる．

★脳梗塞およびTIAの急性期治療および再発予防は，臨床病型に基づいて行われる．

★糖尿病は脳梗塞のリスクファクターであり，特にアテローム血栓性脳梗塞やラクナ梗塞の発症が多い．これまでインスリンなどによる血糖管理は再発予防効果を認めなかったが，インスリン抵抗性改善薬であるピオグリタゾンにて再発予防効果が示されている．
