治療の適応，選択の実際

ポイント

★ALT異常値を示すC型肝炎ウイルス感染者は，原則としてすべて抗ウイルス療法の適応となるが，初回治療例ではウイルス量，genotypeにより治療法を選択する．

★再治療例はペグインターフェロン・リバビリン併用療法が治療の基本であるが，年齢，性別，肝疾患進行度，ウイルス遺伝子変異（コア領域，ISDR），ウイルス量などを参考にし，治療法を選択することが望ましい．

★併用療法の非適応例や無反応例では，進展予防（発癌予防）を目指したALT値とAFP値の正常化，あるいは安定化のための治療法を選択すべきである．
