敗血症の有効な治療はステロイドでしたっけ？―やっぱりEGDT !

ポイント

★感染症によるSIRSが敗血症である．

★初期蘇生はEGDTに従って行い，発症後6時間以内のCVPが8～12mmHg以上，平均血圧65mmHg以上，時間尿量0.5ml／kg以上，中心静脈酸素飽和度70％以上，の達成を目指す^1）．

★抗菌薬は，重症敗血症あるいは敗血症性ショックの認知後，1時間以内に投与する（severe sepsis bundlesは救急室入室時より3時間以内）^2）．

★十分な輸液と血管収縮薬を投与してもショックから離脱できない場合はヒドロコルチゾン（300mg／日以下），バソプレッシン（0.03単位／分）を考慮してもよい^3）．
