脳卒中の一次予防，二次予防

ポイント

★脳卒中の発症には，初発，再発を問わず，高血圧が最大のリスク因子である．

★降圧薬で血圧を厳格に管理することで脳卒中の初発，再発とも予防できることが大規模臨床試験で示されている．

★ACE阻害薬，ARBは降圧利尿薬を併用しなければ十分な降圧が得られず，脳卒中の再発予防効果も得られにくい．

★脳卒中の再発予防効果は降圧度にほぼ依存しており，現在のところいわゆる“J曲線現象”，すなわち過度降圧のリスクは認められていない．
