β遮断薬：もはや主要な降圧薬でない？

ポイント

★高血圧に関する大規模介入試験やメタアナリシスをみると，β遮断薬の心血管イベント抑制効果は多様である．しかし，β遮断薬の効果が他の第一選択薬より劣るとはいえない．

★各種のガイドラインにおいてβ遮断薬は第一選択薬（ESH-ESC2007），高齢者を除き第一選択薬（JSH2009）と評価される一方，第一選択薬より除外するNICE/BSH2006もあり評価は分かれている．

★β遮断薬は頻脈，狭心症，心筋梗塞後は心不全を合併する高血圧患者には積極的適応として推奨されている．

★β遮断薬は喘息，高度徐脈を合併すれば禁忌として使用しない．耐糖能低下，末梢動脈疾患があれば避けるか慎重に使用する．
