非侵襲的陽圧換気（NPPV）―挿管による人工呼吸管理との比較を含めて

ポイント

★NPPVは気管挿管を必要としない人工呼吸である．

★COPD急性増悪と心原性肺水腫による急性呼吸不全ではNPPVの施行が強く推奨される．

★NPPVは適応を選べば，挿管による人工呼吸管理に比べてVAPなどの合併症の発生を低下させ，死亡率・入院日数を減らすことができる．

★NPPV施行後に呼吸状態の改善を認めないときは，速やかに気管挿管への移行を検討しなければならない．
