ポイント

★β遮断薬には徐拍・降圧効果だけでなく，交感神経活性抑制作用による心筋保護や抗酸化作用による動脈硬化の進展抑制やプラーク破綻の抑制などを介した心血管イベントの抑制があり，労作性狭心症や心筋梗塞にはよい適応である．

★冠動脈疾患における予後改善効果は，β₁選択性は影響しないが，内因性交感神経活性がない薬剤や脂溶性薬剤のほうが優れている．

★心筋梗塞においては，発症12時間以内の急性期投与だけでなく，慢性期投与の有効性も示されている．
