CKD患者の貧血治療は益か害か

最近の動向

　腎性貧血は腎不全患者に最も高頻度に発症する合併症で，その第1治療選択薬である遺伝子組み換えヒトエリスロポエチン製剤（rHuEPO）がわが国で実用化してから20年が経過している．rHuEPOを代表とする赤血球造血刺激因子製剤（erythropoiesis stimulating agent：ESA）の登場に伴い，腎性貧血をどのレベルに管理するか（治療の目標ヘモグロビン：Hb）が治療上の大きな課題となった．効果と安全性上，健常者と同じレベルまで貧血を改善するのが得策か否か，の論争である．
