院外心停止の予後改善に向けた取り組み

　私が医師として働き始めた20年前，“DOA（dead on arrival）”と院外心停止はほぼ同じ意味で用いられていました．事実，院外心停止の多くは外来で死亡し，たとえ生存しても意識が回復しない例が大半でした．

　院外心停止の病院前救護は1991（平成3）年の救急救命士法制定により新しい時代を迎えました．2004（平成16）年には一般市民による自動体外式除細動器（automated external defibrillator:AED）の使用が認められ，2007（平成19）年にはおよそ9万台のAEDが医療機関以外の公共施設に設置されたことにより^1），駅やスポーツ施設などでAEDを見かける機会が増えたように思います．
