ポイント

★ACE阻害薬，ARBだけでは，RAA系抑制薬投与初期にアルドステロンブレークスルー現象が認められる．

★心不全の治療において，基礎治療（ACE阻害薬/ARB，β遮断薬）に抗アルドステロン薬の追加投与がとりわけ有効である．

★抗アルドステロン薬は，降圧・利尿作用に加え，臓器保護作用がある．

★エプレレノンは受容体選択性が高く，女性化乳房などの性ホルモンに関する副作用が少ない．
