ポイント

★糖尿病患者では足病変の生涯リスクは25％に及ぶ．適切な予防と治療が必要である．

★糖尿病性足病変に感染が合併している場合には，培養検査や画像評価を行い，抗菌薬や外科的治療を考慮する．

★糖尿病性足病変の初期治療はグラム陽性球菌を対象の基本とし，医療歴や重症度を加味して決定する．

★骨髄炎は再発率が高く治療も長期間に及び，起因菌の同定とデバイスの抜去が重要である．
