脳出血

ポイント

★脳出血の原因の多くは高血圧であり，血圧管理などの内科的治療が主体となる．

★脳出血の外科的治療として，主に救命目的の開頭手術，あるいは，水頭症に対する脳室ドレナージ術がある．

★脳出血急性期では収縮期血圧180mmHg以上，または平均血圧130mmHg以上を目安に降圧を開始するが，降圧目標値に関しては一定した見解は得られていない．

★脳出血慢性期では，国内ガイドラインで示された拡張期血圧75～90mmHg以下，140/90mmHg未満，あるいはより厳しい水準を目標に降圧を行う．
