症例

〔91歳，男性〕

　前年も2回，誤嚥性肺炎で入院した．それぞれ入院期間2カ月以上と長引いている．今回は入院2日前より食欲が低下し，喘鳴も聞かれるようになった．前夜は38.5°の発熱あり，翌日呼吸がひどく苦しそうなので，家族が連れてきて当科外来を受診し，即入院となった．

　意識レベルJCS10，WBC 10,200/μl，核左方移動著明，CRP 28.1mg/dlと激しい炎症所見があり，動脈血ガス分析において，PaO₂ 44.7mmHg，PaCO₂ 31.7mmHg，pH 7.525と急性の呼吸不全があった．入院時，胸部単純X線写真（図1）と胸部CT写真では両側の下肺に肺炎の浸潤像を認めた．
