高尿酸血症は心血管病変のリスクか？

ポイント

★健常者では血清尿酸値はマーカーかリスクかは明らかではないが，ハイリスク患者では血清尿酸値は心血管イベントのリスクであるとする報告が多い．また，低尿酸血症ではかえって心血管イベントの発症リスクが増加するJ-カーブ現象が報告されている．

★高尿酸血症の血管障害の機序として尿酸ナトリウム結晶，酸化ストレスの関与が挙げられるが，さらに近年，尿酸トランスポーターURAT1の関与が注目されている．

★高尿酸血症に対する治療は「6-7-8のルール」を用いる．近年，本邦でも選択的キサンチンオキシダーゼ阻害薬フェブリク^®も発売され高尿酸血症の治療の選択肢が広がった
