虚血性心疾患

　突然始まる胸痛発作のある疾患にangina pectoris（狭心症）という言葉を用いたのは，1768年，Heberdenが最初とされる．その成因が冠動脈疾患であると認識されるようになったのは20世紀に入ってからであり，1940年代になって狭心症と心筋梗塞が独立した疾患として扱われるようになった．冠動脈の内腔狭窄，閉塞の重要性が認識され，その主な成因が粥状動脈硬化であることが明らかにされ，狭心症と心筋梗塞は包括して虚血性心疾患とされるようになった．半数以上が虚血性心疾患である心疾患の死因別粗死亡率は，悪性新生物に次いで第2位を占め増加しているが，年齢調整した虚血性心疾患の死亡率は1970年頃を頂点に減少傾向にある．血行再建治療のめざましい進歩，普及や冠危険因子に対する薬物治療の進歩が貢献していると考えられる．紙面の都合上，本稿では治療を中心にまとめた．
