カンジダ感染症

カンジダ感染症は臨床上非常に重要な疾患であり，頻度の比較的高い腟カンジダ症から，カンジダ血症，侵襲性カンジダ症まで疾患のスペクトラムは広い．また疾患のスペクトラムに合わせて，患者の免疫機能の低下にもスペクトラムが存在する．特に好中球減少時に生じる菌血症は死亡率も高く，診断も困難なこともあり，治療開始のタイミングが議論となる．
主に海外にカンジダ感染症に関して治療ガイドラインが存在するが，おおまかな治療方針は各ガイドラインでおおむね共通していると言ってよい．現在，主なガイドラインとしては2009年にInfectious Disease Society of America（IDSA）からカンジダ感染症の実践的治療ガイドライン1），2012年にEuropean Society of Clinical Microbiology and Infectious Diseases（ESCMID）からカンジダ感染症の診断と治療に関するガイドライン2～6）があり，日本医真菌学会から侵襲性カンジダ症の診断・治療ガイドラインの発表が予定されている（本稿執筆時の2013年5月末日時点では，パブリックコメントの募集が終了している段階でガイドラインの公開はされていないため，ここでは触れない．表1）．IDSAのガイドラインはIDSA監修のもと，和訳もされており，本邦におけるカンジダ症の治療に寄与しているものと思われる．
