ポイント

◎GERD治療の第一選択はPPIである．

◎GERD診療ガイドラインのフローチャートを参考にするとよい．

◎症状に応じて，PPIの投与方法にも工夫をする．

◎PPI抵抗例の原因は，①酸分泌抑制の不足，②酸逆流以外の要因，の2つがある．

◎PPI抵抗例の治療にはPPI倍量や2分割投与，H₂受容体拮抗薬の就寝前投与が有効なことがある．

◎PPI抵抗例へのその他の治療として，消化管運動症機能改善薬や漢方薬，あるいはGABA_B受容体アゴニストが検討されている．
