ポイント

◎腎血管性高血圧は比較的頻度の高い疾患であり，「血圧コントロールに4剤以上の降圧薬を要する例や，治療抵抗性高血圧」「多臓器での動脈硬化症の存在」などをきっかけとして疑い，スクリーニングを検討すべきである．

◎腎動脈ドプラ超音波，CT血管造影（CTA），磁気共鳴血管造影（MRA）などの画像診断が推奨される．

◎高齢者や腎機能障害を認める症例も多いため，造影の検査は指針などを参考に適応を慎重に判断する．
