大腸3D-CT（仮想大腸内視鏡）―大腸がんとガラパゴスシンドローム

最近の動向

　「食生活の欧米化に伴って大腸がんが増えている」と言われて久しい．しかし，近年，大腸がんは欧米より日本で多いのが現状である（図1）^1）．欧米主要国では大腸がんの年齢調整死亡率を右肩下がりに減らしてきたのに対して，日本では頭打ちにはなったものの，まだ高い水準にある．さらに，大腸がん死亡数でみると日本では増加の一途をたどり，2011年には4万5,000人（厚生労働省大臣官房統計情報部「人口動態統計」）を超えた．日本の2.5倍の人口を有する米国では死亡数も年々減少しており，年間5万人程度に過ぎない^2）．このように日本で大腸がんが多い理由の1つとして，諸外国に比べて低い検診受診率が挙げられる（図2）^3～6）．

　日本の大腸内視鏡技術は世界トップレベルである．今後も当面の間，大腸腫瘍の診断・治療の中心は精度が一番高い内視鏡が占めていくだろう．しかし，大腸がん検診の精検受診率は6割程度に過ぎず，2010年度に要精検となった約51万人のうち実に18万4,000人を超える市民が精密検査を受けていない．せっかく大腸がん検診を受けたのに検査陽性者が精密検査を受けない理由はさまざまであるが，「検査がつらそう」という漠然とした不安が一番多いとされている^7）．
