leuko-araiosis（図1）

　大脳皮質下白質，特に脳室周囲白質にCTで低吸収域，MRIのT2強調像やFLAIRにて高信号域が見られることは，高齢者では珍しくない．1987年にHachinskiらがこれを“leuko-araiosis”（＜Greek leukós白質，araios稀薄）と表現することを提唱した．これは“画像上の白質密度低下”を表すことを意図した造語である．当時CT，MRI白質病変の所見に基づきBinswanger病（微小血管の動脈硬化による白質の血管障害に由来する脳萎縮，痴呆）と診断される例が相次いでいたが，病理解剖に基づく疾患頻度から考えて異常な多さであり，画像診断に問題があることは明白であった．このような問題意識を背景に，“白質病変”の病理が明らかになるまでの間，不確かな病理に言及しない純粋な画像所見用語としてleuko-araiosisが提唱された．論文の末尾には“最終的には病理の理解が進むことで，この用語が廃れることを望む”という記載がある．
