神経障害から考える膠原病

ポイント

◎無菌性髄膜炎はウイルス性であることがほとんどであるが，さまざまな自己免疫疾患に起因することがある．診断においては細菌感染の否定が非常に重要である．

◎全身性エリテマトーデス（SLE）やSjögren症候群に関連した横断性脊髄炎が報告されており，そのなかでも近年では抗aquaporin-4抗体陽性の脊髄炎が一群としてまとめられている．

◎血管炎による末梢神経障害は急性〜亜急性のmononeuropathy multiplexのパターンをとるのが特徴である．電気生理学的検査や神経生検を積極的に考慮する．

◎SLEの中枢神経病変は非常に多彩であり，また感染，ステロイドサイコーシス，可逆性後頭葉白質脳症（PRES），血栓性血小板減少性紫斑病（TTP）といったほかの病態と密接に関わっていることがある．

◎神経Behçet病は病変の脳幹・小脳への分布が特徴的で，髄液所見は細胞数および蛋白の中等度以上の上昇を示し，しばしばIL-6の著明な上昇を認める．
