強皮症の病態と最新治療

早期診断の重要性
　強皮症（systemic sclerosis：SSc）は皮膚および内臓諸臓器の線維化，末梢循環障害，抗核抗体産生をはじめとした自己免疫の3主徴を併せもつ膠原病である．近年，多くの膠原病で機能および生命予後の改善が得られているが，SScはいまだ有効な治療法のない難治性病態として取り残されている．その理由の1つとして，病態の中心となる線維化による組織構造の非可逆的な改変が挙げられる．再生能の高い皮膚は，病態進行が止まればある程度改善するが，肺，心，腎，消化管などの内臓機能が一度低下すると回復は望めず，間質性肺疾患（interstitial lung disease：ILD），肺動脈性肺高血圧症，心筋障害が死因として上位を占めている．
　最近，SSc病態を克服するために脚光を浴びているのは，関節リウマチで絶大な成果を挙げた①生物学的製剤をはじめとする分子標的療法の導入と，②早期診断・治療介入である．末梢血，生検皮膚など患者検体やSSc病態を再現した動物モデルを用いた解析によりSScの分子病態の解明が進み，新たな治療標的が次々と同定されている．また，SScでも病変に可逆性が残されている発症早期に的確な治療介入をすることで病変の進行を未然に防ぐ効果が期待されている．
