急性冠症候群のCCU管理

ポイント

◎初期CCU管理の目的は発症直後の致死性不整脈の治療にあり，心電図モニタリング，電気的除細動，一時的ペーシングなどにより，梗塞患者の死亡率を著しく低下させた．

◎Swan-Ganzカテーテルの登場によるポンプ不全の治療の進歩や，早期冠動脈再灌流療法の導入によって治療後合併症の頻度や死亡率が減少した．

◎現在のCCU管理においては，致死性不整脈，ポンプ不全，機械的合併症などに加え，PCIに伴う合併症の早期発見，治療が重要である．

◎虚血巣の縮小や左室リモデリングの抑制のための治療を早期から開始する．
