81歳の男性．当院を受診する2カ月前，発熱と呼吸困難を主訴に前医を受診したところ，肺炎と診断された．セフカペンピボキシルが処方されたものの，症状が持続したため，前医へ入院となった．入院後，抗菌薬はセフトリアキソンへ変更された．治療開始から2週間を経ても症状は改善しなかったが，増悪もしなかったため，在宅酸素療法（安静時3L/分）を導入し，退院となった．その後，自覚症状は自然経過で改善したため，在宅酸素療法は家族の判断で中止した．
しかし，来院6日前より再度37℃台の微熱，咳嗽，呼吸困難が出現した．4日前に前医を受診しファロペネムが処方されたが，症状の改善はなかった．来院当日に前医を再診したところ，呼吸困難とSpO₂の低下（室内気で77％）を認めたため，当院へ救急搬送された．
