30代の男性．1週間前から空腹時の腹痛が続いていた．タール便はない．受診当日の夕方に突然の強い腹痛を認め，救急外来を受診．以前に十二指腸潰瘍の既往がある．診察では上腹部主体の圧痛，筋性防御を認めた．胸部単純X線写真にて右横隔膜下にfree airを認め，続いて腹部単純CTを施行した．
