生来健康でADL自立の40歳男性．入院11日前，雪かき時に転倒し，左橈骨遠位端を骨折．腫脹が強く，手術は延期となっていた．入院当日，回転性眩暈，悪心・嘔吐を主訴に来院し，小脳梗塞の診断で入院．その後，症状は改善傾向．
入院3日目（骨折後14日目）の未明に，臍周囲に軽度の間欠的な腹痛が出現し，同日深夜に症状が増悪したため，当番医がコールされた．
既往歴：高血圧（未治療）以外に特記事項なし．
生活歴：1日20本×22年の喫煙者，飲酒歴なし．1週間前にマグロのたたきを食べた以外には明らかな生物摂取歴なし．
家族歴：母が小脳梗塞の既往あり．
ROS（＋）：臍周囲の間欠痛，悪心，便秘．
ROS（－）：頭痛，気道症状，嘔吐，下痢，血便，排尿症状，関節痛，皮疹．
