インスリン導入総論—基礎が先か，追加が先か？

ポイント
●食事・運動療法や経口血糖降下薬内服にもかかわらず血糖コントロール目標に到達しない場合，インスリン療法の導入を検討する．
●基礎インスリンによるインスリン導入は，追加インスリンに比べて血糖測定・インスリン注射回数が少なく簡便で，体重増加や低血糖リスクも小さい．
●追加インスリンは食後高血糖のコントロールに優れ，内因性インスリン分泌が保たれている症例では空腹時血糖をも制御可能であり，質の高い血糖コントロールに寄与する．
