●Advance Care Planning（ACP）は，終末期患者の医療の質を向上させ，家族のストレスを軽減する．
●終末期医療への関心の高まりとともに，1990年代から米国で発展してきたACPの概念を受け入れる土壌が日本にもできつつある．
●医療者は，終末期を見据えたときの患者のQOLを考慮に入れて，患者本人と家族のディスカッションを上手にファシリテートすることが重要である．
●本稿ではACPの日本語訳として，「先を見据えた終末期ケア意思共有」を提案したい．
