微小脳出血（CMB）の意義

ポイント
●微小脳出血（CMB）はT2^＊強調画像上の直径10mm未満（通常は5mm未満）の円形または楕円形の低信号域として描出され，脆弱な細動脈壁から流出した陳旧性出血を示唆する．
●CMB保有群は非保有群と比較して，頭蓋内出血（ICH）発症リスクが高まる．
●CMBを複数有する症例では，抗血栓療法によるICHのリスクが高いと予測される．
●降圧管理をはじめとした危険因子管理と，CMBの存在を確認したうえで，ICHのリスクを考慮した最適な抗血小板療法・抗凝固療法・血栓溶解療法を選択する必要性がある．
