ニューモシスチス肺炎（PCP）

はじめに
　ニューモシスチス（Pneumocystis）は1909年にChagasによって発見され，その後，1980年代にHIV感染の流行とともに日和見感染症の起因菌として広く認識されることになった．近年ではPCP（Pneumocyctis pneumonia）予防薬とART（antiretroviral therapy）によりHIV患者のPCP（HIV-PCP）は減少傾向にあるものの，種々の免疫抑制薬使用の増加に伴いnon-HIV患者のPCP（non-HIV-PCP）は増加傾向にある．HIV-PCPとnon-HIV-PCPの間には臨床的に大きな違いがあり，それぞれの特徴を示した（表1）．Non-HIV-PCPに関するエビデンスは，HIV-PCPと比較して乏しいが本稿ではそれを踏まえて，日常臨床における現実的なマネジメントを示したい．
