腎盂腎炎の原因微生物は大腸菌が多いと思いますが，最近ESBL産生菌，さらにCREといったものを聞きます．これらは何でしょうか？
ESBL産生菌とは
　すべてのペニシリン，セファロスポリン，モノバクタムの分解能を有するβラクタム系抗菌薬分解酵素である，基質特異性拡張型βラクタマーゼ（extended spectrum β-lactamase：ESBL）を産生する細菌のことを指す．緑膿菌やアシネトバクターなどのブドウ糖非発酵菌がESBLを産生する例も報告されてはいるが，臨床的・疫学的に主に問題となるのは大腸菌，肺炎桿菌などのクレブシエラ属菌，Proteus mirabilisなどの腸内細菌科細菌である．本来これらはほとんどのβラクタム系抗菌薬に感受性があるが，接合伝達性プラスミドを介してESBL遺伝子を獲得することでESBL産生菌に変化しうる．かつては，ESBL産生菌は主に院内で分離される肺炎桿菌で認められていたが，近年は，外来患者から分離される大腸菌においてもしばしば認められるようになった^1）．
