人工呼吸器関連肺炎—マネジメントとその予防

Point
◎人工呼吸器関連肺炎（VAP）の発生頻度は高く，合併した場合の致死率は9〜13％と高い．
◎VAPの主な原因は上気道や消化管細菌叢の下気道への落ち込みである．
◎VAPの診断は総合的な臨床判断に基づく．臨床肺感染症スコア（CPIS）が有用である．
◎晩期VAPでは耐性菌が問題となることが多い．
◎VAPの予防として，半座位管理，手指衛生，口腔ケアが重要である．
