抗酸菌症の最新の考え方—ジェネラリストが抗酸菌症を診る際のdo's and don'ts

Point
◎肺結核・肺非結核性抗酸菌症（肺NTM症）は珍しい疾患ではない．肺結核患者は減少しているが，今後，肺NTM症患者はさらに増加することが予想されている．
◎臨床的に肺結核が疑われた場合には，喀痰の抗酸菌塗抹を3回提出し，感染性に関して判断する．
◎キノロン系抗菌薬の安易な処方は，肺結核の診断の遅れにつながる．
◎肺NTM症が鑑別に挙がった場合には，少量のクラリスロマイシンやアジスロマイシンの投与は行わない．
◎肺NTM症において空洞を有する症例や病変の範囲が広い症例は，早期に専門医への紹介を検討する．
