非痙攣性てんかん重積

Point
◎意識障害患者において，脳波異常がみられるてんかん患者が稀ならず存在する．
◎最近のガイドラインでは「可視的な症状（発作）の有無によらず，臨床的あるいは電気的（脳波で確認できる）てんかん活動が少なくとも5分以上続く場合，あるいはてんかん活動が回復なく反復し，5分以上続く場合」をてんかん重積状態（SE）と定義している．
◎SEは，全身痙攣が主体の痙攣性てんかん重積状態（CSE）と，痙攣を伴わない非痙攣性てんかん重積状態（NCSE）に分類される．また，抗てんかん薬2剤により適切に治療してもてんかん発作が治まらないSEを難治性てんかん重積状態（RSE）と分類する．
◎持続脳波モニタリングは，集中治療中においても脳波異常の確認からNCSEの診断と治療に有用である．
